Getting your PSC or PSR report from the ESRD-QIP website

Your facility’s Performance Score Certificates (PSC) are available after December 15th for download on the ESRD-QIP website. The current PSC will remain available online until the start of the PY (Payment Year)review period next summer.  ESRD-QIP instructions are listed below.

Since you are required to post a paper copy in a prominent patient area throughout the year, we recommend that you download the PDF file so that paper copies can be created as needed before the document goes away next summer.

Due to the high volume of users requesting reports via the ESRD-QIP website, you may experience some delays in retrieving your reports. Once you request a report please allow two hours for the system to generate your report. If within two hours your requested report is not retrieved, then submit a helpdesk ticket providing your CCN and the desired report name to .

The Final Performance Score Report (PSR) will be available for download after the first of January. An announcement will be made when those are available.

Downloading the ESRD-QIP PSR or PSC 

While this uses the PSR, which is not yet available, as an example, the same process is used for requesting and downloading the PSC.

Please note: If you do not have a staff member with the Facility Point of Contact (POC) role for ESRD-QIP in QARMS, then you will not be able to retrieve your reports. Only the ESRD-QIP POC can generate and retrieve the reports.
