Advance Care Planning: Protect your family by thinking ahead and making your decisions known

Advance care planning not only includes advance directives, like a living will or power of attorney, but it also includes considering what you would do when you have to decide on a new medication, surgery, or new living arrangements, just to name a few examples. Planning for the future is not an easy thing to do but it can really help you and your family have peace of mind when the time comes. Long term care planning can help prepare for the possibility of receiving help at home, or moving to assisted living or nursing home living. Advance funeral planning can help you be secure in knowing your wishes are known and your family will not be placed in a position to make difficult decisions when you cannot. Not only will you be sharing your wishes, you will prevent your family from having to deal with additional burdens while they grieve if something were to happen to you. HEALTH DECISION TOOLS

Ottowa Hospital Research Institute offers decision making tools for specific medical conditions, including Kidney Disease.

o    The Ottawa Personal Decision Guide (OPDG) is designed for any health-related and/or social decisions. It can help people assess their decision making needs, plan the next steps, and track their progress in decision making. This resource is available http://decisionaid.ohri.ca/index.html and is offered in multiple languages with several different versions for individuals and families.  English Version / Spanish Version

Another Resource is Decision point- Advance Care Planning: Should I Stop Kidney Dialysis which is an online tool you can use regarding: 1) Get the Facts; 2) Compare Options; 3) Identify Your Feelings; 4) Quiz Yourself and provides a Summary on your answers when considering stopping dialysis.

The Eldercare Locator, a public service of the Administration on Aging, U.S. Department of Health and Human Services, is a nationwide service that connects older Americans and their caregivers with information on senior services.  1-800-677-1116 or www.eldercare.gov/ELDERCARE.NET.  English Version / Spanish Version 

http://longtermcare.gov  is another public services of the Administration on Aging, U.S. Department of Health and Human Services, that shares information on long term care planning including planning basics and how to decide and pay for care.
