Quality Insights Renal Network 5 is pleased to invite dialysis providers to participate in the 2019 5-Diamond Patient Safety Program. Now in its 12th year, the program is designed to assist dialysis facilities in improving both staff and patient awareness of specific patient safety topics, using web-based educational modules to address all aspects of patient safety. Each module includes objectives, required activities, tools and resources, and optional activities. For each module successfully completed, the facility is awarded a “Diamond,” culminating in special recognition for 5-Diamond facilities.

New for In 2019…

  • Quality Insights Renal Network 5 is partnering with the dialysis team at Texas Children’s Hospital to revise each module to include a pediatric perspective.
  • Synaptiv will continue to provide continuing education credit to nurses for completion of the Culture of Safety in-service.
  • The National Nephrology Certification Commission (NNCC) will continue to accept completed 5-Diamond Patient Safety Program modules toward CCHT recertification requirements. Details are available on the 5-Diamond website at http://5diamondpatientsafety.org/About/Guidelines.aspx#CE.

The program is open to all facilities nationally and is endorsed by the American Nephrology Nurses Association (ANNA), the National Renal Administrators Association (NRAA), the Renal Physicians Association (RPA), the American Association of Kidney Patients (AAKP), Dialysis Clinic, Inc. (DCI), Fresenius Medical Care (FMC), and US Renal Care.
